ABSTRACT

ABSTRACT BACKGROUND: New endoscopic treatments for gastroesophageal reflux (GERD) are developed every year and are indicated in cases that are refractory to conventional therapies as well as after surgical treatment failure. OBJECTIVE: To present the first cases of endoscopic therapy for GERD performed in Brazil. METHODS: Use of radiofrequency with the Stretta procedure in symptomatic volunteers diagnosed with GERD. RESULTS The technique was performed in three patients after they were included in the study protocol. No patient had complications, and all patients were discharged on the same day, either without medication or taking it sporadically for symptom control. CONCLUSION: Endoscopic treatment for GERD using radiofrequency was effective in the cases presented herein with no technical complications.


RESUMO CONTEXTO: Novos tratamentos endoscópicos para refluxo gastroesofágico são desenvolvidos a cada ano, sendo indicados em casos refratários às terapias convencionais, bem como após a falha do tratamento cirúrgico. OBJETIVO: Apresentar os primeiros casos de terapia endoscópica para tratamento do refluxo gastroesofágico realizado no Brasil. MÉTODOS: Uso de radiofrequência com o procedimento de Stretta em voluntários sintomáticos e diagnosticados com DRGE. RESULTADOS: A técnica foi realizada em três pacientes depois de terem sido incluídos no protocolo de estudo. Nenhum paciente teve complicações, e todos receberam alta hospitalar no mesmo dia, mantendo-se sem medicação ou fazendo uso esporádico para o controle de sintomas. CONCLUSÃO: Tratamento endoscópico para doença do refluxo gastroesofágico com uso de radiofrequência foi eficaz nos casos aqui apresentados e sem complicações técnicas.

BACKGROUND: The aims of this study are to evaluate the efficacy and safety of intragastric balloon (IB) to reduce the weight and body mass index (BMI) in severely obese adolescent females and to describe the changes in the liver enzymes and lipid and glucose metabolism biomarkers. METHODS: This study included 10 severely obese post-menarche adolescent females. We evaluated anthropometric data, lipid profile, glucose metabolism biomarkers, and liver enzymes before insertion and after removal of the IB. RESULTS: BMI and weight reduction were larger during the first month of intervention. Especially in the first week, there was a reduction of 1.74 ± 0.46 kg/m2(p= 0.004) and 6.46 ± 1.52 kg (p= 0,002), respectively. After 3 months, there wasan average BMI reduction of 4.29 ± 1.04 kg/m (p= 0.005) and weight reduction of 12.9 ± 3.08 kg (p= 0.004). From the initial moment to study conclusion, there was a statistically significant reduction in insulin levels (9.0 ± 2.8 U/mL; p=0.012) and in homeostatic model assessment-insulin resistance (2.0 ± 0.6;p= 0.009). Five patients reported, during the first week, epigastric pain; nausea and vomiting were observed in two patients. No adolescents presented dysphagia during IB use. CONCLUSION: IB use in adolescent females with severe obesity in association with a conservative multidisciplinary treatment had a positive impact in BMI reduction, with few adverse effects. There was also an improvement in insulin resistance.

ABSTRACT Background: Less invasive and complex procedures have been developed to treat obesity. The successful use of Endoscopic Sleeve Gastroplasty using OverStitch(r) (Apollo Endosurgery, Austin, Texas, USA) has been reported in the literature. Aim: Present technical details of the procedure and its surgical/ endoscopic preliminary outcome. Method: The device was used to perform plications along the greater curvature of the stomach, creating a tubulization similar to a sleeve gastrectomy. Result: A male patient with a BMI of 35.17 kg/m2 underwent the procedure, with successful achievement of four plications, and preservation of gastric fundus. The procedure was successfully performed in 50 minutes, time without bleeding or other complications. The patient presented mild abdominal pain and good acceptance of liquid diet. Conclusions: The endoscopic gastroplasty procedure was safe, with acceptable technical viability, short in duration and without early complications.


RESUMO Racional: Procedimentos menos invasivos e complexos têm sido desenvolvidos para o tratamento da obesidade. A gastroplastia vertical endoscópica com uso de OverStitch(r) (Apollo Endosurgery, Austin, TX, EUA) já foi relatada com sucesso na literatura. Objetivo: Apresentar detalhes técnicos do procedimento e seu resultado cirúrgico/endoscópico preliminar. Método: O equipamento foi utilizado para realizar plicaturas ao longo da grande curvatura gástrica, objetivando tubulização do estômago semelhante a gastrectomia vertical. Resultado: O método foi aplicado em paciente com IMC 35,17 kg/m2, sendo realizadas quatro plicaturas, preservando o fundo gástrico. O procedimento foi realizado com sucesso em 50 min, sem sangramento ou outras complicações. O paciente evoluiu com dor abdominal leve, e teve boa aceitação de dieta líquida. Conclusão: A realização da gastroplastia endoscópica foi segura, com viabilidade técnica aceitável e reprodutível, com curto tempo de procedimento, sem complicações precoces.

ABSTRACT

Metastasis is an infrequent cause of tumor involvement of the major duodenal papilla and of cholestasis. We report a case of obstructive jaundice due to a metastatic breast carcinoma to the major duodenal papilla, and assess curative and palliative treatment that interventional endoscopy can offer.


La metástasis es una causa infrecuente de compromiso de la papila mayor y colestasis. Reportamos una causa rara de ictericia causada por metástasis de cáncer de mama en la papilla mayor duodenal.

Dois experimentos foram realizados para avaliar o efeito de duas formulações comerciais à base de Beauveria bassiana (Bals.) Vuillemin e Metarhizium anisopliae (Metsch., Sorokin sobre os parâmetros biológicos de Trichogramma atopovirilia Oatman & Platner, 1983. No primeiro experimento, cartelas com ovos de Spodoptera frugiperda, 1797 foram mergulhadas em suspensões preparadas com os produtos e, em seguida, colocadas à disposição dos parasitóides para oviposição, durante um período de 24 horas. No segundo, fêmeas do parasitóide foram alimentadas com uma solução mel e suspensão de conídios. Em ambos os experimentos as fêmeas mortas foram colocadas em uma câmara úmida para observar a esporulação dos fungos. Os tratamentos foram mantidos em câmara climatizada com temperatura de 25±1º C, umidade relativa de 70±10 por cento e fotofase de 14 horas. Avaliou-se a longevidade e mortalidade dos adultos, índice de parasitismo, emergência do parasitóide, número de indivíduos por ovo e razão sexual dos descendentes. As formulações não interferiram nos parâmetros avaliados e não foi observada a esporulação do fungo no cadáver do parasitóide adulto. É possível inferir que T. atopovirilia e os fungos entomopatogênicos B. bassiana e M. anisopliae são compatíveis e podem ser empregados simultaneamente em programas de manejo integrado de S. frugiperda.


Two bioassays were performed to evaluate the effect of two biopesticides based on Beauveria bassiana (Bals.) Vuillemin and Metarhizium anisopliae (Metsch.) Sorokin on the biological parameters of Trichogramma atopovirilia Oatman & Platner, 1983. In the first one, displays with S. frugiperda, 1797 eggs were dropped into the biopesticide suspension and offered to the parasitoid females for 24 hours. In the second one, parasitoid females were fed with a suspension containing honey and biopesticide suspension. In both cases, after the parasitoid death they were mantained into a humid chamber to observe fungus sporulation. The experiments were maintained in a climatized chamber at 25±1º C, RH 70± 10 percent, and photophase of 14 hours. The longevity and adult mortality, total parasitism, progeny emergency, number of individuals per egg and sexual ratio were analyzed. The biopesticides did not affect the evaluated parameters and no fungus sporulation was observed in dead females. It is possible to assert that the entomopathogenic fungi B. bassiana and M. anisopliae can be used with T. atopovirilia in IPM S. frugiperda systems.

ABSTRACT

O carcinoma pancreático habitualmente é de diagnóstico tardio, sendo o tratamento endoscópico aplicado à intenção paliativa. Relata-se um caso de paciente feminino, 36 anos, apresentando tumor recidivado em cabeça de pâncreas após submeter-se à pancreatectomia caudal associada a esplenectomia, quimioterapia adjuvante e evolução tardia com quadro de estenose antroduodenal e de colédoco distal. Procedeu-se à passagem de prótese metálica auto-expansível na via biliar e através de estenose antroduodenal. A paciente evoluiu com melhora clínico-laboratorial e sobrevida de quatro meses

ABSTRACT

PURPOSE: To perform a endoscopic gastrostomy by the introducer method with routine instruments used in a general hospital, without special instruments of special kits.METHODS: This procedure was performed in pigs (Sus scrofa domesticus) under observation for seven days and then submitted to euthanasia. The technique was evaluated for macroscopic and histologic parameters. RESULTS: All animals had a good evolution without major complications. Some minor complications ocurred like a rupture of Foley catheter balloon and subcutaneous space abscess.CONCLUSION: The percutaneous gastrostomy with routine general hospital instruments is sucessful performed, is safe, cheap and must be performed by skilled endoscopists.
